Job Description

Job Title: Community Liaison

Status: Non-Exempt; Union

Reports to: Case Manager

Date: 12/16

Basic Function:

Under direction, is responsible for recruiting and engaging community residents, program participants, community organizations and service agencies to become Infant Mortality Ambassadors.

Responsibilities:

· Under direction, ensure that the activities of this position are consistent with the mission, vision and values of Merrick House.

· Promote the MomsFirst Project mission throughout the city of Cleveland.

· Coordinate and facilitate all meetings of the Neighborhood Consortia.

· Maintain calendar of community events at your site.

· Staff community events.

· Support MomsFirst activities in the community.

· Facilitate partnerships with public and private service providers to meet gaps in services identified by participants, staff and consortia members.

· Build relationships with neighborhood business, i.e. corner stores, barber shops, etc., to increase referrals into the Project.

· Attend monthly Community Liaison meetings. Participate in Ohio Equity Institute meetings and others as scheduled.

· Provide feedback regarding the progress and challenges of outreach, community engagement efforts during Community Liaison meetings and/or as requested.

· Canvass the neighborhood to identify and enroll pregnant women into the MomsFirst Project.

· Maintain documentation and reports as required. For example, record quotes/feedback from residents.

· Carry a caseload of high-risk pregnant women providing education, support and referrals to meet the need of the participant service plan ensuring monthly programmatic benchmarks are met for all participants.

· Complete participant care plans and screenings.

· Assist in securing participant involvement in program events, meetings, activities and the Ohio Equity Institute meetings.

· Maintain accurate and up to date documentation on all participant activity in the paper case file and data system.

· Participate in ongoing continuing education and trainings.

· Exemplify positive role modeling by professionalism, language, work ethic and positive communication.

· Perform other duties as assigned

Requirements:

· Combination of education and experience represented by an Associate’s Degree in Early childhood Development, social Work.

· Three to five years’ experience as a community based volunteer, outreach worker or community organizer preferred.

· Must be proficient in the Healthy Start Community Health Worker Competencies.

· Excellent communication, organization, and documentation skills required.

· Proficiency in Microsoft office word processing software (WORD), e-mail software (Outlook) and navigating the internet.

· Must have a good working knowledge of infant mortality.

· Knowledge of factors related to racial and ethnic health disparities preferred.

· Non-traditional work hours required.

· Must have valid Ohio Driver’s License, reliable means of transportation for local travel and car insurance.

· Must have good dexterity with the ability to lift up to 30 pounds, push, pull, carry, climb, balance and kneel and the ability to sit, stand, walk, bend, and squat.

· Must have good oral, written, and interpersonal communication skills.

· Ability to work with diverse client population, community organizations as well as staff at all levels.

· Must be well organized, detail oriented and have the ability to maintain confidentiality.

· Must be able to work independently from general instruction.

· Physical examination by a licensed physician required including a TB test.

· Final applicant is required to be fingerprinted to complete background check.
